LIST OF DOCUMENTS REQUIRED FROM FOREIGNERS APPLYING FOR ADMISSION TO FIRST-DEGREE STUDIES AT THE SGH UNDER RULES OTHER THAN BINDING FOR POLISH CITIZENS

Admission to first-degree studies requires the submission of the following documents:

1. signed personal questionnaire;
2. application to the SGH Rector for admission to studies;
3. Photocopy of passport or other administrative identification document certified for conformity with the original by the SGH or a consul of the Republic of Poland;
4. photocopy of a visa or a residency card or another document entitling its holder to remain within the territory of the Republic of Poland, certified for conformity with the original by the SGH or a consul of the Republic of Poland;
5. photocopy of the insurance policy in case of illness or a personal accident insurance policy for the period of education in Poland or a European Health Insurance Card, certified for conformity with the original by the SGH, a declaration of a foreigner applying for admission to studies that he/she will take out an insurance with the National Health Fund immediately upon commencing education;
6. medical certificate confirming lack of contraindications to undertake studies together with a certified translation into Polish, prepared by a certified translator entered in the list of translators by the Ministry of Justice of the Republic of Poland or certified by a consul of the Republic of Poland;
7. 2 current photographs with the size of 37 x 52 mm and 1 full-colour photograph in digital form;
8. a Polish secondary school-leaving certificate or a certificate or other document obtained abroad, entitling them to apply for admission to studies at universities of all kinds in the country in whose system the institution granting the certificate operates, such certificate being considered equivalent to the Polish secondary school-leaving certificate pursuant to the regulations on nostrification of school certificates and secondary school-leaving certificates obtained abroad or considered equivalent to the Polish secondary school-leaving certificate or considered as entitling its holder to undertake such studies in the Republic of Poland under an international agreement;
   1. legalised or endorsed with an apostille (the legalisation requirement does not apply to the International Baccalaureat and European Baccalaureat diplomas)
   2. certified translation of the certificate into Polish, prepared by a certified translator entered in the list of translators by the Ministry of Justice of the Republic of Poland or certified by a consul of the Republic of Poland in office in the country where the document was issued;
   3. certificate confirming the equivalence of the foreign certificate with the Polish secondary school-leaving certificate, issued in conformity with the regulations on the nostrification of school certificates and secondary school-leaving certificates obtained abroad (the above certificate should be submitted within the deadline not exceeding the end of the first semester of studies).

ADDITIONAL DOCUMENTS FOR STUDIES IN ENGLISH
Foreingers applying for admission to studies conducted in the English language submit a document confirming the knowledge of the English language (one of the listed):

1. Certificate in Advanced English (CAE), Certificate of Proficiency in English (CPE), Certificate in English for International Business and Trade (CEIB); 
2. Test of English as a Foreign Language (TOEFL) - at least 87 points in the Internet-Based Test (iBT) version or at least 180 points in the Computer-Based Test (CBT) version; 
3. Spoken English Test (SET) for Business: Stage C "Mastery" level; 
4. London Chamber of Commerce and Industry (LCCI): English for Business Level 3, English for Business Level 4.
5. International English Language Testing System IELTS - over 6 points
6. Certificate, diploma or another document confirming graduation from institution at least on secondary level where classes were conducted in Enligh. 

All accepted documents confirming the knowledge of the English language are listed in Annexe no. 2 to the Ordinance of the President of the Council of Ministers dated 16 December 2009 on the mode of conducting the qualification proceedings in civil service (Journal of Laws (Dz.U.) No. 218, item 1695). The above mentioned documents may be submitted in the form of a photocopy of the document, certified for conformity with the original by the SGH or a consul of the Republic of Poland.

ADDITIONAL DOCUMENTS FOR STUDIES IN POLISH
Foreigners applying for admission to studies conducted in the Polish language, except the persons referred to studies, submit one of the documents confirming knowledge of Polish:

1. certificate of completion of a year-long preparatory course for commencing education in Polish, conducted at entities identified by the minister competent for higher education, 
2. holding a certificate of the knowledge of the Polish language, issued by the State Commission for the Certification of Proficiency in Polish as a Foreign Language, 
3. qualification test on the knowledge of the Polish language taken at SGH before a commission appointed by the SGH Rector,
4. Polish secondary school-leaving certificate where studies were conducted in the Polish language,
5. higher education diploma from university where studies were conducted in the Polish language,

The above mentioned documents may be submitted in the form of a photocopy of the document, certified for conformity with the original by the SGH or a consul of the Republic of Poland.
Foreigners holding a Polish Card [Karta Polaka] submit a photocopy of the said document, certified for conformity with the original by the SGH or by a consul of the Republic of Poland.
Foreigners of Polish origin pursuant to Article 5 sec. 1-3 of the Act on Repatriation dated 9 November 2000 (uniform text Journal of Laws (Dz.U). of 2004, No. 53, item 532 as amended) submit a photocopy of the decision confirming their Polish origin, certified for conformity with the original by the SGH or by a consul of the Republic of Poland. 
In the case of foreigners who are to undertake studies on the basis of a referral addressed to the SGH from institutions (the Bureau for Academic Recognition and International Exchange (“BUWiWM") or a head of the entity in which the foreigners referred by BUWiWM undertook the preparatory course in the Polish language, authorised by the minister competent for higher education) it is required to submit:

1. referral from the entity where the foreigner underwent a preparatory course in the Polish language (drafted in conformity with the template established by BUWiWM) together with a photocopy of the letter from BUWiWM to the said entity, confirming the qualification of the foreigner for studies in Poland, or
2. direct referral from BUWiWM addressed to the SGH.

LIST OF DOCUMENTS REQUIRED FROM FOREIGNERS APPLYING FOR ADMISSION TO SECOND-DEGREE STUDIES AT THE SGH UNDER RULES OTHER THAN BINDING FOR POLISH CITIZENS

Admission to second-degree studies requires the submission of the following documents:

9. a copy of the higher education diploma drafted by a Polish university issuing the said diploma, or a photocopy of the said diploma´s original certified by the SGH or a foreign diploma or another university graduation document authorising its holder to undertake second-degree studies in the country of its issue:
   1. legalised or endorsed with an apostille (original);
   2. certified translation of the foreign diploma into Polish, prepared by a certified translator entered in the list of translators by the Ministry of Justice of the Republic of Poland or certified by a consul of the Republic of Poland in office in the country where the diploma was issued;
   3. nostrification certificate or copy of the resolution of the SGH Senate on exempting the foreigner from the nostrification procedure on account of applying for admission to a specific field of second-degree studies at the SGH: in the case of a foreign diploma or document which is not acknowledged under an agreement on mutual recognition of education documents binding on the Republic of Poland (the abovementioned nostrification certificate or copy of the resolution of the SGH Senate should be submitted not later than by the end of the first semester of studies).

ADDITIONAL DOCUMENTS FOR STUDIES IN ENGLISH

Foreingers applying for admission to studies conducted in the English language submit a document confirming the knowledge of the English language (one of the listed):
